Periodontal olarak riskli dişlerin direkt kompozit ile restorasyonu: Olgu raporu

GİRİŞ: Bu çalışmanın amacı, protruziv pozisyondaki üst lateral dişin estetik olmayan görünümünü güncel adeziv rezin kompozitler kullanarak minimal invaziv olarak tek seansta restore etmektir.
OLGU RAPORU: Yetişkin kadın hasta üst lateral dişi ile ilgili estetik kaygıyla kliniğimize başvurdu. Klinik, radyografik ve intraoral muayene yapıldı. 12 numaralı dişin periodontal hasara bağlı mobilitesinin olduğu görüldü. Hastaya uygun tedavi seçenekleri anlatıldıktan sonra 12 numaralı dişe rezin kompozit veneer yapılması kararlaştırıldı. Renk seçiminden sonra anestezi uygulanıp rubber dam ile izolasyon sağlandı. Protruziv pozisyondaki sağ üst lateral kesici diş, estetik olarak doğru konuma yerleştirilmek için prepare edildi. 12 ve 11 numaralı dişler arasındaki diastema 11
numaralı dişin formu genişletilerek kapatıldı. Ayrıca 11 numaralı dişin insizal kenarı 21 numaralı diş ile aynı hizada olacak şekilde restore edildi. Diş preparasyonundan sonra mine yüzeyine %35'lik fosforik asit (K-Etchant Syringe, Kuraray, Japonya) 30 saniye uygulandı ve 30 saniye durulanıp, hava ile hafifçe kurutuldu. Tek şişe bonding ajan (Clearfill S3 Bond Universal, Kuraray, Japonya) uygulandı, 20 saniye polimerize edildi. Dişler nanohibrit kompozit (Estelite Asteria A2B, Tokuyama Dental, Japonya) kullanılarak restore edildi. Alüminyum oksit grit diskler (SoFlex, 3M ESPE, ABD) kullanılarak bitirme ve parlatma protokolleri uygulandı. Restoratif tedavi tamamlandıktan sonra periodontal splint uygulandı.
SONUÇLAR: 6 aylık takip sonucunda estetik sonuçlar korunmuştur. 12 numaralı diş vitalite testlerine pozitif yanıt vermiştir. Direkt kompozit restorasyonlar, tek randevuda tamamlanması nedeniyle daha az maliyet ve zamanda iyi estetik sonuçlar ortaya çıkarmaktadır.

Restoration of periodontally compromised teeth with direct composite: A case report

INTRODUCTION: The aim of this study is to remove the unaesthetic appearance of protrusive positioned lateral tooth and restore it minimally invasively with adhesive resin composites.
CASE REPORT: A female patient applied our clinic with aesthetic concerns. Clinical, radiographic and intraoral examination were performed. Tooth numbered 12 was periodontally damaged and had mobilty. Other treatment options were explained to the patient. The patient preferred to have adhesive resin composite veneer restorations. After color selection, anesthesia was applied and isolation was provided with rubber dam. The left upper lateral incisor tooth was prepared to replace the correct position to generate aesthetic appearance. The diastema between teeth 12 and 11 was closed by enlarging the form of 11. Also the incisal edge of tooth number 11 was restored in line with tooth number 21. After tooth preparation %35 phosphoric acid (K-Etchant Syringe, Kuraray, Japan) was applied on enamel surface 30 seconds and rinsed 30 seconds and dried with air slightly. Single bottle bonding agent (Clearfill S3 Bond Universal,Kuraray, Japan) was applied and polymerized for 20 seconds. Then tooth was restoreted using nanohybrid resin composite (Estelite Asteria A2B, Tokuyama Dental, Japan) according manufacturers instructions. Aluminum oxide grit discs (SoFlex, 3M ESPE, USA) were used for finishing and polishing protocols. After restorative treatment was completed, periodontal splint was applied.
CONCLUSION: After 6 months of follow-up, the aesthetic results remained stable. The tooth number 12 had a positive response to the vitality test. Direct composite restorations provides good aesthetic result in a single session at less cost and time.
