CERRAHİ HASTALARINDA PREHABİLİTASYON: CERRAHİ HEMŞİRELİĞİNİN GELİŞEN ROLÜ

Özet

Giriş: Cerrahi girişimler, organizmada fizyolojik stres yanıtına ve katabolik süreçlere neden olarak postoperatif iyileşmeyi olumsuz etkileyebilmektedir. Prehabilitasyon, hastanın cerrahi öncesi fizyolojik ve psikososyal rezervini optimize ederek cerrahi strese toleransını artırmayı ve postoperatif sonuçları iyileştirmeyi amaçlayan multimodal bir yaklaşımdır. Bu süreçte cerrahi hemşiresinin değerlendirme, eğitim, danışmanlık ve bakım koordinasyonu rolleri giderek önem kazanmaktadır.

Amaç: Bu derlemenin amacı, cerrahi hastalarında prehabilitasyonun temel bileşenlerini ve klinik etkilerini incelemek ve prehabilitasyon sürecinde cerrahi hemşiresinin gelişen rolünü ortaya koymaktır.

Yöntem: Çalışma geleneksel derleme yöntemiyle gerçekleştirilmiştir. PubMed, Web of Science, Scopus ve Google Scholar veri tabanlarında “prehabilitasyon”, “cerrahi hastaları”, “cerrahi hemşireliği” ve “perioperatif bakım” anahtar kelimeleri kullanılarak literatür taranmıştır. Güncel araştırmalar, sistematik derlemeler, meta-analizler ve rehberler incelenerek elde edilen veriler sentezlenmiştir.

Tartışma: Literatür, egzersiz, beslenme, psikolojik destek ve medikal optimizasyonu içeren multimodal prehabilitasyonun fonksiyonel kapasiteyi destekleyebildiğini, postoperatif komplikasyonları azaltabildiğini ve iyileşme sürecine katkı sağlayabildiğini göstermektedir. Cerrahi hemşiresi ise risklerin erken belirlenmesi, bireyselleştirilmiş eğitim ve danışmanlık, izlem, yönlendirme ve multidisipliner koordinasyon aracılığıyla programın sürekliliğinde merkezi bir rol üstlenmektedir.

Sonuç: Prehabilitasyonun perioperatif hemşirelik bakımına sistematik olarak entegre edilmesi, cerrahi hemşiresinin ameliyat öncesi risk azaltma ve iyileşmeyi destekleme rolünü güçlendirebilir. Kanıta dayalı ve bireyselleştirilebilir hemşirelik protokollerinin geliştirilmesine ihtiyaç vardır.

Introduction: Surgical procedures can negatively impact postoperative recovery by causing physiological stress responses and catabolic processes in the organism. Prehabilitation is a multimodal approach aimed at improving surgical stress tolerance and postoperative outcomes by optimizing the patient's pre-surgical physiological and psychosocial reserves. In this process, the roles of the surgical nurse in assessment, education, counseling, and care coordination are becoming increasingly important.

Aim: The aim of this review is to examine the fundamental components and clinical effects of prehabilitation in surgical patients and to highlight the evolving role of the surgical nurse in the prehabilitation process.

Method: This study was conducted using a traditional review method. Literature was searched using the keywords “prehabilitation”, “surgical patients”, “surgical nursing”, and “perioperative care” in PubMed, Web of Science, Scopus, and Google Scholar databases. Data obtained by examining current research, systematic reviews, meta-analyses, and guidelines were synthesized.

Discussion: The literature shows that multimodal prehabilitation, including exercise, nutrition, psychological support, and medical optimization, can support functional capacity, reduce postoperative complications, and contribute to the recovery process. The surgical nurse plays a central role in the continuity of the program through early identification of risks, individualized education and counseling, monitoring, referral, and multidisciplinary coordination.

Conclusion: Systematically integrating prehabilitation into perioperative nursing care can strengthen the surgical nurse's role in preoperative risk reduction and supporting recovery. There is a need for the development of evidence-based and individualized nursing protocols.
