Histopathological Features of Conjunctiva Tumours

Abstract

Objective: To evaluate the histopathological features of conjunctival tumors. Methods: The medical records of patients who have undergone surgery for conjunctival tumors at Sivas Cumhuriyet University Faculty of Medicine, Department of Ophthalmology between 2006 and 2021 were evaluated retrospectively. Results: A total of 124 patients with histopathological diagnosis were included in the study. The mean age of the patients was 40.5±26.2, of whom 53 (42.7%) were female and 71 (57.2%) were male. The localization of the tumors were on the conjunctiva in 106 (85.5%) patients, on the limbus in 13 (10.5%) patients, and on the caruncle in 5 (4%) patients. Of the conjunctival tumors, 7 (5.6%) were congenital, 28 (22.6%) were premalignant/malignant tumors of the surface epithelium, 45 (36.3%) were melanocytic, and 18 (14.5%) were stromal. Benign conjunctival tumors were observed in 94 (75.8%) patients with a mean age of 31.623 years. The most common benign conjunctival tumor was nevus in 43 (34.6%) patients, pyogenic granuloma in 15 (12%) patients, and dermoid in 6 (4.8%) patients. Premalign/malign conjunctival tumors were observed in 30 (24.1%) patients with a mean age of 68.410.6 years. Of these, 27 (21.7%) were conjunctival intraepithelial neoplasia (CIN), while 2 (1.6%) patients had primary acquired melanosis and 1 (0.8%) patient had squamous cell carcinoma. Positive surgical margin was demonstrated in 6 patients with premalign/malign tumors. Intraoperative mitomycin-c (MMC) was applied to 13 patients and postoperative 0.02% MMC was prescribed topically to all of these patients 4 times a day for 14 days. Alcohol epitheliopathy was performed in 2 patients and cryotherapy was performed in 1 patient. No recurrence was observed in any of the patients followed. Conclusion: In our study, conjunctival tumors were mostly detected as benign lesions, while the most common benign tumoral lesion was nevus. The most common premalign tumoral lesion was CIN. Premalign/malign conjunctival tumors were observed in patients with an older mean age compared to benign tumors (p<0.001).
